Family Assessment Form

FAF Express is an abbreviated version of the FAF Pro. Distinguishing features include:

FAF ratings are entered in a spreadsheet format.
Available reports are limited to family functioning reports and a case log report.
Intake data is entered only for the adult caregivers in the family.
FAF Express does not include the ability to assign workers to cases, develop service plans, attach contact notes to cases, or create a closing summary report including statistics related to amount of service provided a family.

FAF Express might be preferable for agencies when:

They have a limited number of computers available for staff use.
They are using FAF strictly as an outcome measurement tool and not an assessment and service planning tool for workers.
They have other required means for collecting intake data, documenting service plans and contact notes, and writing closing summaries.
They prefer to have their workers use the FAF paper version and have a data entry person enter FAF rating data.
